there are many cases where prosecutions have taken place - have you tried looking at the All England Law Reports? remembering the Act is dated 1974 - there is a whole lot of other case law which is pertinent to defining health and safety law ie reasonably practicable etc -the NEBOSH Diploma focuses on these case laws - happy to forward more information if you need.
