I agree with James. Its important to install this importance to staff for their own good as well as for the company and therefore they need to get over the "its only a" attitude and be made to feel comfortable about filling in the book/sheet.
This comes down to staff training and easy access to the method of reporting so that it becomes something that is not a big deal. Rather a over reporting culture than under.
