The HSE has launched a prosecution against Norfolk waste company, Enviro-Waste Ltd, after an investigation into the deaths of three employees.
The company is accused of failing to manage the risks of slurry-holding tank. Three employees died in the incident in Thetford, when they were overcome by fumes in a 12ft slurry tank in July 2004.
