A final report into the Hatfield rail crash, which was carried out by an independent investigation board set up by the HSE, has criticised both Balfour Beatty and Railtrack.
Four people were killed and more than 70 injured when a London to Leeds express train derailed south of Hatfield station in Hertfordshire in October 2000.
