On 26 June 1997, a gas explosion at the Kerrin Point tower block
owned by Lambeth Council in south London, caused extensive damage to
the block and surrounding property. The explosion occurred during
refurbishment work to the hot water boiler system The HSE's
investigation into the accident concluded that all the parties to the
work (the Council, contractor, Staward Engineering Limited, and
subcontractor, Mr Duffel) failed to appreciate the scale of risk
