A worker has been crushed to death between a forklift truck and a lorry in a horrific accident at an East Anglian farm. The 48-year-old man, Kevin Smith, worked for Anglia Grain Services and was delivering grain to Mulley's Farm in Little Bromley, Essex, when the accident happened.
