Two companies have been successfully prosecuted for failing to ensure machinery is properly isolated before cleaning or maintenance, following the deaths of two workers.
The two employees of Galloway and MacLeod Ltd, Peter Brown and Charles Lee Hinshelwood, were killed on 26 November 2005 when they were cleaning inside an animal feed mixer which started up unexpectedly and without warning. They were found fatally injured inside the mixer.
