I am looking to design a stadard template which will allow me to discharge my responsibilities as a Health and Safety Manager regarding issues I have identified that no actions have been taken either/or my advise has been ignored due to other constraints....
I'm looking for terminology and/or written statements...

With regard to the Noise at Work Regulations, area of interest "Annual Hearing Tests for employees exposed to Noise", what criteria is used in the hearing test?

I understand that a business may give payment in lieu of notice to a member of staff.
Is it possible for a member of staff to make a payment in lieu of giving notice in the same way?
The contract requires 3 months notice by either party.
I am looking at this from a business continuity/succession planning perspective.
