If in doubt whether, always, often, sometimes, rarely or alomost never in terms of "when" rather than "if" something is likely to happen try reverse engineering the process and check with HSE on statistics on whatever hazards associated with your industry and what the 'consequences' are.
Involve the very people at risk, they tend to be far more aware of the risks they take on a daily bassis, and are closest to the hazards they encounter.
If you are engaged in the process and mitigation is agreed by both parties identification and action taken to reduce status of hazard (when not 'if' ) then neither party has delegated responsibility solely to the other.
Then do not need to be a Hawkins type mathamatician with an 'ology' to come up with over complex algorythemes remembering the old question "does your dog bite?" ooouch - "that's not my dog!"

Will the introduction of 'more' recognised accreditation impede the process by which some genuine health and safety advocates wish to gain a legitimate foothold and start practicing as consultants. Surely potential clients would vet the competence of such consultants very much in the same way an employer would or should vet it's subcontractors for competence and compliance.
