An oil firm, ConocoPhillips Limited, has been ordered to pay more than £1 million in fines and costs at Grimsby Crown Court for breaching health and safety rules in connection with two incidents in 2001, including a huge explosion at the Humber Refinery, South Killingholme, North Lincolnshire. The incident was described as “possibly the most serious chemical incident in Britain since the Flixborough disaster”.
