HMRC data loss incident was "entirely avoidable"

The report into the loss of 25 million data records by Her Majesty’s Revenue and Customs (HMRC) has found that the incident was “entirely avoidable”, and happened because of “serious institutional deficiencies” rather than because of one person’s actions.
